carta notarial debe contener lo siguiente:<\/li><li>El texto: \u201cCARTA NOTARIAL\u201d, en la parte superior del documento.<\/li><li>El nombre y direcci\u00f3n del destinatario y del remitente.<\/li><li>La direcci\u00f3n del destinatario debe estar detallada de forma precisa y concreta; y, de ser necesario, se debe se\u00f1alar o anexar una referencia.<\/li><li>Debe estar foliada fecha de salida y retorno).<\/li><li>De ser el caso, presentar la copia del DNI del acompa\u00f1ante del menor.<\/li><\/ul><\/li><\/ul><ol><li>Si se encuentra representado por un apoderado, se deber\u00e1 presentar el certificado de vigencia de poder actualizado, expedido por Registros P\u00fablicos (SUNARP).<\/li><\/ol><\/div>\n\t\t\t\t\t\t\t\t\t<div class=\"elementor-tab-title tabindex=\"0\" hidden=\"hidden\"><ul><li style=\"list-style-type: none;\"><ul><li>Copia del DNI del menor.<\/li><li>DNI de ambos padres.<sup> 1<\/sup><\/li><li>Si el menor ha sido reconocido solo por uno de sus padres, basta el consentimiento de este.<\/li><li>Si uno de los padres ha fallecido, deber\u00e1 presentar la partida o acta de defunci\u00f3n.<sup> 2 <\/sup><\/li><li>Informaci\u00f3n relevante del viaje (itinerario, fecha de salida y retorno).<\/li><li>De ser el caso, presentar la copia del DNI del acompa\u00f1ante del menor.<\/li><\/ul><\/li><\/ul><ol><li>Si se encuentran representados por un apoderado, se deber\u00e1 presentar el certificado de vigencia de poder actualizado, expedido por Registros P\u00fablicos (SUNARP).<\/li><li>El documento debe ser ORIGINAL y ACTUALIZADO, con una antig\u00fcedad no mayor a tres meses desde su emisi\u00f3n.<\/li><\/ol><p><strong><u>NOTA<\/u><\/strong>: La autorizaci\u00f3n de viaje se visa en el Colegio de Notarios
